South Central Family Health Center is recruiting for the following positions:

Family Physician (2 Full-time position)

RESPONSIBILITIES:
 To provide a full spectrum of family practice services including prenatal and pediatrics.  Perform procedures including minor surgery, flexible sigmoidoscopy, colposcopy, basic wound care and laceration repair. Supervision of mid-level practitioners required’d.

QUALIFICATIONS: Possession of M.D. or D.O. degree from an accredited university, valid unrestricted CA medical license, completion of residency in family medicine from an accredited program, BE/BC, valid DEA, and clinical experience working with diverse populations.  Knowledge of quality assurance standards helpful. Must be bi-lingual/bi-literate Eng/ Span. 


 Licensed Clinical Social Worker (1  Full-Time position)

RESPONSIBILITIES: To identify, triage, and manage patients with mental health and behavioral health conditions in a primary care model.  Work with primary care team to treat and manage patients with chronic emotional and/or health problems efficiently and effectively. Educate patient, families, and staff self-management, prevention, and treatment enhancement techniques. Provide and/or coordinate staff training to ensure staff competency. Attend and participate in Quality Assurance activities as required.

QUALIFICATIONS:  Masters’ Degree in social Work or Psychology. Current State of CA licensure.  Minimum 2 years experience in health related field.  Must have excellent interpersonal skills and desire to work in a community clinic environment with diverse population.  Must be bilingual/bi-literate English/Spanish and a team player.

 


Registered Nurse ( 1- Full-Time Position)

FUNCTIONS: To provide quality health care to a culturally diverse population.

 

RESPONSIBILITIES:  To provide triage (patient assessments), return patient phone calls, dispense medication (under direction of contracted pharmacist), and other patient care task as directed by RN Supervisor.  Other duties may be assigned as required by RN Supervisor.

 

QUALIFICATIONS: Licensure as Registered Nurse.  Two to four years’ community clinic experience required.  Must have excellent patient interaction skills and a desire to work in a community clinic environment with diverse population.  Knowledge of quality assurance standards helpful.  Bi-lingual/bi-literate English/Spanish preferred.
