South Central Family Health Center accepts all patients regardless of their insurance or financial status.
Our federally funded health care program allows us to offer a wide range of services to patients through a sliding-fee scale; we also take most employment-based insurances.
Eligibility
South Central Family Health Center offers a sliding fee scale to all income eligible uninsured or under-insured patients based on annual household income for all of our services. The income guidelines and acceptable verification for the sliding fee are listed below.

Medi-Cal Eligibility
SCFHC and HPFHC Managed Care Enrollers are available to assist our uninsured patients. They have been trained to help you understand the insurance plans, what they cover, and whether you qualify for them. They will also help you fill out the paperwork to enroll in a plan. We make every attempt to help patients find coverage.
Please bring with you the following items to speed up the processing of your paperwork
- California ID or Consulate ID
- Residency card (if applicable)
- Social Security Card
- Birth Certificate
- 2 current pay check stubs (if paid in cash, please bring a letter written by your employer as certification of employment)
- Automobile registration (if applying for Medi-Cal)
- Proof of address (utility bill: gas, water, or light bill )
