Enhanced Care Management

    A holistic patient-centered, interdisciplinary approach to care that considers the (non)clinical needs of the most vulnerable community members.

    Enhanced Care Management (ECM) is a FREE added benefit for Medi-Cal beneficiaries:

    ECM services are offered in person and via Telehealth.

    If interested, ask front desk staff or your medical provider to refer you to the ECM Program at SCFHC.

    • Homelessness or at risk
    • Avoidable hospital or ER use risk
    • Mental Health and/or substance use disorder needs.
    • Transitioning from Incarceration or Youth Correctional Facilities
    • At risk for long-term care
    • Institutionalization Nursing Facility (or SNF) Residents transitioning to the Community
    • Enrolled in California Children’s Services
    • Involved in Child Welfare
